OUR STORY
Century Homecare was founded in 2012 to provide homecare services to underserved communities. At Century, we aim to provide care not only post hospitalizations, but also provide mental health care, and care for chronic health conditions. We continue to serve maligned communities such as the poor, immigrants, and non-English speakers in addition to the traditional homecare recipients. We take a holistic approach to caring for our patients. A high percentage of the patients we service have other social economic factors that impact their health, like access to food, transportation for their medical appointments, language barriers. We address barriers to healthcare access by partnering with families and health care providers to build a patient-focused community around each patient.
Our AFC program focuses on providing in home support for individuals who need assistance with activities of daily living by a live in caregiver. Our members due to a medical or mental health condition require hands on (physical) assistance or cueing and supervision to complete ate least one of the following activities: Bathing, Dressing, Toileting, Transferring, Mobility and Eating.
