ADULT FOSTER CARE (AFC) PROGRAM:
Century AFC offers a MassHealth-funded program designed to provide in home support for individuals who need assistance with daily tasks by a live-in caregiver. Century AFC is an accredited program by CARF and an approved provider by MassHealth to offer this critical, compassionate and essential service. Members enjoy the warmth and comfort of being cared for at home by a trusted caregiver while the caregivers are rewarded for this invaluable service with a tax-free stipend. Both members and caregivers benefit from continuous support and education from our multidisciplinary team of professionals, ensuring that everyone involved is well equipped and informed.
AFC ELIGIBILITY CRITERIA
ELIGIBLE MEMBERS:
MassHealth- eligible members 16 years of age or older with insurance coverage, who meets the clinical eligibility criteria to qualify for AFC services and has a live-in caregiver, and whose Primary care physician (PCP) has ordered AFC.
CLINICAL ELIGIBILITY CRITERIA FOR AFC
- AFC must be ordered by member’s PCP
- The member has a medical or mental health condition that requires daily hands-on (physical) assistance or cueing and supervision throughout the entire activity in order for the member to successfully complete at least ONE of the following:
- Bathing
- Dressing
- Toileting
- Transferring
- Mobility (ambulation inside and outside the home)
- Eating
- Must reside in an AFC qualified setting located in Massachusetts
AFC CAREGIVERS:
- A responsible person who is at least 18 years old
- Be able to provide the needed personal care to the member and ensure member’ safely and well-being at all times
- Can NOT be a “Family Member- a spouse, parent of a minor member, including adoptive parent, or any legally responsible relative of the member”
- Meet all other requirements for an AFC caregiver

MEMBER ASSESSMENT AND CARE COORDINATION:
Century AFC program integrates regular assessments by our skilled nurses and care managers to continuously monitor the overall health and care needs of our members. This vigilant system of observation and reporting by the AFC caregiver and our multidisciplinary team ensures that each care plan remains adaptable to the evolving needs of every member.
If our assessment identifies a need for additional services, we are proactive in facilitating that care. Our nurses or care managers are at the forefront, providing referrals and assisting members in setting up these supplemental services, which may include:
- Adult Day Care
- Skilled Nursing and Therapy services at Home
- Nutritional assistance programs such as food banks or Meals on Wheels
- Dental care
- Eye care and other specialized medical services
